docs(20-01): plan 20-01 complete — migration 0009 applied to prod

client_transcripts table verified in production: 7 columns (id, lead_id,
client_id, title, content, call_date, created_at). Applied via SSH tunnel.
Plans 20-02 and 20-03 unblocked.
